Heart Attack

A heart attack is a serious medical event that happens when the blood supplying oxygen to the heart is cut off, usually because of a blockage in an artery. This results in damage or death of part of the heart muscle. The heart can heal, but like any part of the body that was injured it leaves a scar. This scar tissue does not contract effectively, so the ability of the heart to pump is lowered after a heart attack.

A heart attack, sometimes called a myocardial infarction, can happen without warning. However there are certain factors that raise the risk of heart attack. Some of these are:

  • Age
  • Gender (most heart attack sufferers are male)
  • A family history of heart attacks
  • Smoking
  • Obesity
  • High cholesterol
  • High blood pressure
  • Diabetes
  • Unhealthy lifestyle choices (lack of exercise, poor diet, too much alcohol or stress)

Heart attacks typically feel like pain or pressure in the chest that can radiate out to the back, arm, or jaw. Some other symptoms can include fatigue or weakness, shortness of breath, sweating, and nausea.

Treatment aims to increase blood flow to the heart muscle by opening up blocked blood vessels. Medications are sometimes used to dissolve clots in the arteries. Aspirin and other medications such as Plavix can thin the blood and make clots less likely to form. Dilators can widen blood vessels to make room for more blood to flow. Doctors can also use a procedure called catheterization to look at the blood vessels around the heart directly, place stents, or widen vessels using angioplasty. In extreme cases bypass surgery can be used to help blood flow better. After recovery, it is important to lead a healthy lifestyle and take any prescribed medications in order to protect the heart.

Heart Problems

The heart is one of the most important organs in the body. This smooth muscle expands and contracts rhythmically an entire lifetime, pumping blood to the lungs and then to every other cell in the body. When heart problems occur, it becomes difficult for the body to get the nutrients and oxygen it needs via the blood. So while there are a variety of illnesses and disorders that affect the heart, most of them cause weakness, exhaustion and shortness of breath. The most common heart conditions include heart disease, angina, arrhythmia, and valve disorders.

Heart disease is the leading cause of death in the United States for both men and women. This includes coronary artery disease, heart attacks, congestive heart failure, and congenital heart disease. Some conditions, such as genetics, cannot be controlled. But there are many other things one can do to lower their risk for heart disease. Controlling high blood pressure and cholesterol levels, reducing or stopping smoking, exercising more and losing weight if needed, and eating a diet low in sodium can all protect the heart.

Angina is a squeezing type of chest pain that happens when the muscles around the heart don't get enough oxygen. It can be regular or infrequent. Usually, angina is caused by coronary heart disease. However, not all chest pain is angina. Chest pain can also be caused by a lung infection or panic attack, for example, so it is important to have any sudden pain checked.

Arrhythmia is the word for when the heart beats too fast, too slow, or irregularly with skipped beats. It can feel frightening, and depending on the type it can be dangerous, but in most cases arrhythmia is not serious and can be treated. It is very common, especially in older adults.

Heart valve problems can happen in any one of the the heart's four valves that keep blood flowing where it needs to go. Babies can be born with problems in their heart valves, or valves can be damaged by infections. The valves can stiffen and become less mobile, or they can stop closing properly and 'leak' when the heart beats. Valve problems can usually be repaired surgically.

Percutaneous Left Atrial Appendage Closure (LAAC)

Percutaneous left atrial appendage closure (LAAC) is a device implantation procedure that reduces the risk of stroke for patients who have atrial fibrillation, a condition in which the heart beats out of rhythm. Atrial fibrillation causes blood to collect in the left atrial appendage, a small sac located in the top left chamber of the heart. The blood that collects in this appendage can form clots and cause stroke when pumped out of the heart.

To implant the device, a long, flexible tube, called a catheter, is inserted into a large vein in the groin and advanced to the heart. Once the tube reaches the left side of the heart, X-ray is used to guide a thinner catheter into the left atrial appendage. The tiny device is then passed through the tube and into the appendage. When the doctor has made sure it is in the right place, she takes the catheter out, and the procedure is completed.

This minimally invasive procedure usually requires a hospital stay of at least one day. Normal activities may be resumed within a few days. About 45 days following implantation, a test will be done to determine whether the device has closed the left atrial appendage. Check-ups have to be performed every year to make sure the device is in place.

International Heart Institute of Missoula and Dr Musco have been great. For my procedure I felt taken care of by great nurses and the staff for the Ablation procedures (though for that I was unconscious so can't be 100 percent sure there) .. The only caveat if recommending to someone else is the hospital there is down one operating room for repairs and so things are booked a long ways out. And it is a super busy clinic...while I felt taken care of, it was also obvious things move quickly in there and bed space at a premium so you need to ask questions and let them know if any concerns etc..be proactive if possible. But it was good care over all for sure, and the fact they travel once a month to different towns for follow up appts is golden. I had to travel there for the procedure, but my follow ups if nothing goes awry in the meantime (knock on wood) can be done when they are next in my town. Love that. Thanks to Dr Musco and staff at the clinic and St Pats/Providence. Hoping this procedure does the trick and leads to a better life and lifestyle. Cheers.

I am happy to have Dr Musco as my provider for my heart condition. Exponentially better than my cardiology experience in my home town so far. That said, everyone is busy and seems heart things are only getting busier. I had a good 3 month checkup with Dr Musco...but scheduling was nit great this time. It was supposed to be an in person visit in my town, same as occurred after my first procedure.. However something got changed or my appt was given to someone else or the like, as much later I was called and told Id either need to come over to the clinic for the appt or do it online. It could be just a patient priority and mine was not high risk, I dunno I just rolled with it. But preferable would have been to have my 3 mo checkup in person in my town like last time..that was my expectation but the appt got changed. Just worth mentioning is all.. Life goes on, and again, I'm very happy to be in the care of Dr Musco and his staff for what's been a scary condition at times for me. Cheers.

Everyone from the receptionist, MA, and Dr. Musco were very kind and professional. The appointment went smoothly. All of the staff were attentive and explained things in ways I can understand. The plan of care was clear and Dr. Musco was very prepared and explained the next steps clearly. He had excellent bedside manner and was very kind and professional. He put my mind at ease and I could tell he really cared and practicing medicine was not just a job for him but his calling. I will definitely recommend IHI and St. Pat's to friends and family. I had many concerns and questions about a recent diagnosis, I Was very nervous about a procedure suggested by another physician so I asked for a referral. Dr Musco spent time and went over all my tests and labs and my health history with me. Gave me opportunity to ask questions and gave me my treatment options explaining them I feel very thoroughly. I felt very comfortable with Dr. Musco, also that he cared about me as a patient, had gathered more information about my health history than I've seen on MyChart, so that was really good. He was very thorough with explanations, and listening. I wish there were an easy way to make corrections (in what I supposedly said) to the notes in my chart. But other than that, I am very happy with the care provided to me by Dr. Musco.

Dr Musco was able to fit me into a full schedule and spend almost an hour discussing my complicated type of atrial fibrillation. He went through 5 written questions in detail and we achieved a "shared decision making process" that I thought was remarkable. I am very grateful that he is my cardiologist. I am a retired internist and have never had a fellow physician explain issues and answer specific question so well.

I appreciated Dr. Muscos time and effort in getting familiar with my case before my appointment. I am a complex case and he relieved the stress of me trying to explain my history. My appointment was the best experience I could have hoped for. My Dr was amazing and I felt heard and understood for the first time since I developed heart issues. I will drive from Great Falls to stay with him as my Dr. Im so relieved! Just wish I could have gotten in much sooner.
