Headache

Headaches are dull or sharp pains that occur in regions of the head and face. Headaches occur in many different forms and vary in location, severity, and duration. They are not necessarily a sign of an underlying illness and often resolve on their own. However, headaches can present significant day-to-day discomfort. The most common forms of headaches include:

Migraines: one of the most prevalent types of headache, migraines typically last from two hours to three days, causing a throbbing pain on one side of the head. Nausea, sensitivity to bright lights, fatigue, and blurred vision all may accompany migraines. While migraines have no known cure, medications and lifestyle modifications may ease symptoms. Avoiding behaviors that may trigger migraines such as lack of sleep or loud, repetitive noises can improve headaches.
Tension headaches: also known as stress headaches, these headaches are characterized by pain in the upper top part of the head and behind the eyes. Tension headaches are common and have no exact cause; however, certain practices may increase the likelihood of developing them. For example, wearing one's hair up in a tight hairstyle or wearing a restrictive hat can put pressure on the scalp, leading to a tension headache. Other causes of tension headaches may include alcohol or nicotine use, caffeine, teeth grinding, stress, overexertion, and eye strain. Some people find posture improvement, alternating sleeping positions, and neck exercise can alleviate tension headaches. Over-the-counter pain relievers such as ibuprofen (Advil, Motren) and naproxen sodium (Aleve) may also reduce tension-headache pain.
Sinus headaches: this type of headache causes pain in the sinuses, but may progress to pain and pressure in the eyes, forehead, and cheeks. These headaches are often accompanied by nasal congestion, fatigue, and ache in the upper teeth and may or may not be caused by sinus infections or sinusitis (sinus inflammation). Some sinus headaches are allergic headaches, which arise from allergic triggers such as seasonal allergies (pollen allergies) or hay fever, causing sinus pressure and facial pain. Other symptoms of allergic headaches include nasal congestion, irritated or watery eyes, runny nose, and sneezing. Allergic headaches may be treated with antihistamine medications such as cetirizine (Zyrtec). Allergic headaches are common during pollen season, which is usually around springtime.
Cluster headaches: these headaches occur suddenly and cause stabbing, sharp pain from one eye down to the one side of the neck. They are often referred to as the most aggravating and painful type of headaches. Other symptoms of cluster headaches include nasal congestion, droopy eyelid, and enlarged pupil on one side of the face. In the past, it was widely believed that cluster headaches were caused by a bevy or "cluster" of multiple types of headaches, such as a combination of an allergic headache and a tension headache. Medical experts now recognize that cluster headaches are a separate and unique form of headache. Cluster headaches last for around twenty minutes to two hours. They may recur throughout the period of a day or several weeks, but usually dissipate within a month. Treatment for cluster headaches includes over-the-counter pain medications such as ibuprofen (Advil, Motren) and naproxen sodium (Aleve). Prescription Prednisone can help with swelling and Verapamil may reduce pain by relaxing the blood vessels.

Living with headaches is challenging; headaches can prevent people from fully enjoying life. Medical treatment and care in avoiding certain triggers can help those with headaches begin to feel normal once again.

Mononucleosis (Mono)

Mononucleosis, frequently called ''mono,'' is a viral infection that causes fatigue, fever, swollen lymph glands, and sore throat. The Epstein-Barr virus (EBV), spread through saliva, is the primary virus that leads to mono. Mono is most common in teenagers and young adults, where EBV is often transmitted through kissing or sharing drinks. For this reason, mono is colloquially referred to as ''the kissing disease.'' Mononucleosis usually resolves on its own; however, recovery from mono can take from several days to several weeks. Some young adults may continue to experience extreme fatigue from mono for months, often requiring a semester home from school.

Mononucleosis happens in three stages:

During the initial stage, those with mono may experience few symptoms as EBV replicates in the body. Symptoms such as headaches and body aches are often misattributed to allergies or stress. People with mono may continue performing their daily activities and attending school, leading to increased transmission of the virus to others. The initial stage lasts for about five to fourteen days.

During the acute stage, mono symptoms intensify and may include the enlargement of the spleen, a sore throat, fatigue, liver issues, and skin rashes. The mono infection is at its worst in this stage.

Lastly, the convalescent stage is when recovery begins. This period of healing can last from two to six months.

Mono can be detected by a physical exam; physicians can diagnose mono by checking for inflamed lymph nodes, tonsils, or spleens. Blood antibody tests may also help diagnose mono by revealing antibodies in the blood that were formed as a response to the Epstein-Barr virus. Elevated levels of white blood cells can also suggest that a patient has mono.

There are no medications specifically for mono or the Epstein-Barr virus, and patients are advised to rest and avoid strenuous exercise. Drinking plenty of fluids, eating a healthy diet, and taking anti-inflammatory ibuprofen (motrin) may also help people who are afflicted with mono feel better.

People with mono should take precautions to avoid spreading the condition to others. They can prevent infection by not sharing glasses, food, and utensils with others until they fully recover.

Thyroid Problems

Thyroid problems are a group of conditions that alter hormone regulation and metabolism. The thyroid is a small, butterfly-shaped gland located in the lower neck, just below the Adam's apple (laryngeal prominence) and larynx (voice box). Despite its small size, the thyroid has an outsized effect on the body by regulating hormones that control metabolism. Metabolism is the body's process of converting consumed food into fuel to power the entire body. Heart rate, weight, cholesterol, body temperature, and even physical growth and development are all affected by the metabolism.

When the thyroid malfunctions, the body is unable to properly process and allocate energy. Thyroid problems, such as hyperthyroidism (overactive thyroid) and hypothyroidism (underactive thyroid), cause irregular metabolism. In some cases, the body produces antibodies that attack the thyroid cells for unknown reasons. The damaged thyroid responds by releasing either too little or too much T3 and T4 thyroid hormones. Thyroid malfunctions may also arise from genetic causes or iodine deficiency. Thyroid problems may include:

Hashimoto's thyroiditis, an autoimmune disease that causes hypothyroidism
Graves' disease, an autoimmune disease that causes hyperthyroidism
Goiter
Thyroid nodule
Thyroid cancer

Hashimoto's is the leading cause of hypothyroidism, which is when the thyroid does not produce enough hormones to stimulate adequate metabolism. The condition is diagnosed by blood tests that measure TSH (thyroid stimulating hormone) and detect Hashimoto's antibodies. Symptoms of hypothyroidism include sluggish metabolism, inflammation, fatigue, weight gain, brittle hair, dry skin, and sensitivity to cold. To supplement impaired thyroid function, patients with Hashimoto's are typically prescribed synthetic thyroid hormone pills. Such patients may need to consume synthetic thyroids for the rest of their lives. Other treatments may include dietary changes, such as the avoidance of certain foods and medication to reduce inflammation. Frequent blood tests can inform patients if their TSH levels are within a normal range.

Grave's disease is most frequently the cause of hyperthyroidism, which is when the thyroid produces too many hormones, overstimulating one's metabolism. The condition is also diagnosed by blood tests that measure TSH and detect Grave's antibodies. Elevated heart rate, high blood pressure, sweating, bulging eyes, and weight loss are common symptoms of hyperthyroidism. Treatments for hyperthyroidism include oral radioactive iodine, which slowly shrinks the thyroid, slowing the production of excess thyroid hormone. Other anti-thyroid medications such as methimazole (Tapazole) also reduce the production of thyroid hormones. Blood pressure medications known as beta blockers can ease the increased heart rate caused by hyperthyroidism. If treatments are unsuccessful, patients may be prescribed a thyroidectomy, or surgery to remove part or all of the thyroid. Some patients with hyperthyroidism may require ophthalmological (eye medicine) treatments such as eye drops, prednisone, and surgery to reduce eye swelling and bulging.

Untreated thyroid disorders frequently lead to goiters or thyroid nodules. A thyroid goiter is a large lump that can be felt near the base of the neck and is made of swollen thyroid tissue or nodules (excess cell growth). Nodules and goiters are diagnosed by CT scan or ultrasound. Obstructive goiters can cause pain, coughing, and abnormal breathing. The most common cause of goiters is hyperthyroidism (overactive thyroid). However, Hashimoto's is also known to cause goiters or nodules. Goiters develop from continued damage from antibodies to the thyroid over time. At times, a goiter can result from a cancerous thyroid tumor. Most goiters and nodules are themselves benign.

The exact causes of thyroid cancer are unknown. People who develop thyroid cancer may or may not have another thyroid problem such as Hashimoto's or Grave's disease. Symptoms of thyroid cancer include a nodule that can be felt on the neck, hoarseness, difficulty swallowing, swelling in the neck, and neck and throat pain. CT and MRI scans are used to diagnose thyroid cancer. There are four types of thyroid cancer which vary in intensity. Thyroidectomy (thyroid removal surgery) is usually prescribed to treat thyroid cancer.

Thyroid problems are often long-term conditions that require some form of treatment. With regular monitoring and medication, many people with thyroid problems are able to lead active and fulfilling lives.

There are so many good aspects of the Providence Health organization and my doctor, Michael Caldwell. The My Chart system has been excellent for me. It is always operational, well organized and a good way for me to message with Dr. Caldwell who is very responsive and helpful. Through that recently, it was determined that I needed to come for an office visit. I was impressed that Dr. Caldwell was able to be seen within 36 hours of that communication - I know people with other health care provider that wouldn't not be able to be seen in such circumstances for weeks. This is a testament to Providence's dedication to care quality - making sure Dr. Caldwell is available to provide care when needed. Furthermore, he remains an excellent provider - knowledgeable, a good listener, friendly and efficient. For this reason, I am always happy to see him when he has medical students training under him - he is an excellent example for future health care professionals.

My PCP is on sabbatical. When I called and explained my concerns the schedular attempted to talk to a nurse. When one wasnt readily available she left a message for them. The nurse called me back within the hour and advised me they had spoken to the on-call provider and instructed me to call right at 8 am the next morning to get in same day. I was scheduled to see Dr. Caldwell. He was very professional and so kind during the visit. He had a medical student who took my history and then she presented my case to him with me. He was very complimentary to her work and very respectful in his feedback. I appreciated the interaction. It took a little more of his time and I think it was a valuable experience for his student. I truly appreciate his clinical presentation and all of his explanations of the course of treatment planned.

I had never seen Dr. Caldwell before but I thought I might have an infection in my toe and the scheduler got me in right away when I called the office. Dr. Caldwell is an extremely nice man and very carefully checked my toe and gave me very detailed instructions on the care I should take to clear up the infection, also a prescription which I am taking. He also personally talked to the schedular to make sure I had a follow up appointment with my regular doctor and made sure I knew to call them if there was any change, and what to look for in a change. I have been with Providence/Western mt. Clinic for many years and due to their care I have made it longer than I thought I would have.

Always a great experience with Dr. Caldwell and his staff! We were discussing A1C and medication use to bring it into appropriate range. Recommended counseling and medication that would help in meeting the appropriate goals.Dr. Caldwell is always helpful and concerned with your medical needs but is never an alarmist. He persuades and counsel you and tells you the reasons for the pathway he is taking with your health. If you have questions he listens and answers in terms and information you can understand.I have been Dr. Caldwell's patient for many years and have always valued his medical expertise highly.

I have been a patient of Dr. Caldwell's for over 25 years. His level of care, knowledge about my health and history, up to date medical knowledge, empathy and concern, working collaboratively with me around health solutions, responsiveness to any urgent needs, communication and follow-up has been exemplary. I absolutely love Dr. Caldwell. He always greets me with a smile and a hand shake. He is aware of my medical history because I have seen Dr. Caldwell for about the past 20 years. He takes good care of me and my health. He has a great sense of humor and is happy and has good bedside manner. I have recommended Dr. Caldwell to my 80+ year old mother. She is happy working with Dr. Caldwell. He takes good care of her too. My daughter with disabilities also sees Dr. Caldwell. An awesome doctor. We need more like him for sure! ) Thank you!

Dr. Caldwell is always very pleasant and professional. My only complaint is that I waited for over 45 minutes waiting for my appointment in the waiting room. During that time, I heard one of the receptionists tell another patient that if a patient is 5 minutes late to his appointment, then the physician wouldn't see him. Yet- a provider In that practice can make the patient wait over 45 minutes to even get called back? That seems a bit one-sided to me...my time is just as valuable as any clinic's or provider's time.

This doctor was the "on-call" doctor. I had a sore knee due to a fall and needed to know what to do and if I could go ahead with plans I had made. He examined the knee, explained his diagnosis, and gave me procedures to follow that he believed would alleviate the pain. I followed them to the letter, and within less than a week the pain was almost entirely alleviated. I went ahead with my plans, with no ill effects. I am grateful to him for giving me the tools I needed to make that possible.

Dr. Caldwell is a trusted and valued health care provider for my husband and myself. He is always ready to give us advise on medical questions, no matter if it at an office call or when running into us socially. If the question requires more than a short response he suggest we come to the office. He always reviews our charts and suggests changes to our prescriptions, futher testing or anything we need. He makes me feel he is really invested in my health. Great doctor!!!

I wish I could let the administration know that the care I receive at Providence is above and beyond expectations. Dr. Caldwell has been my doctor for 20 some years...he and every other provider I have been in contact with is over the top professional, caring, and incredible...and that includes the office people, xray techs, nurses and physicians. I would like to find a way to give back for the wonderful care I recieve.
