Thyroid Problems

Thyroid problems are a group of conditions that alter hormone regulation and metabolism. The thyroid is a small, butterfly-shaped gland located in the lower neck, just below the Adam's apple (laryngeal prominence) and larynx (voice box). Despite its small size, the thyroid has an outsized effect on the body by regulating hormones that control metabolism. Metabolism is the body's process of converting consumed food into fuel to power the entire body. Heart rate, weight, cholesterol, body temperature, and even physical growth and development are all affected by the metabolism.

When the thyroid malfunctions, the body is unable to properly process and allocate energy. Thyroid problems, such as hyperthyroidism (overactive thyroid) and hypothyroidism (underactive thyroid), cause irregular metabolism. In some cases, the body produces antibodies that attack the thyroid cells for unknown reasons. The damaged thyroid responds by releasing either too little or too much T3 and T4 thyroid hormones. Thyroid malfunctions may also arise from genetic causes or iodine deficiency. Thyroid problems may include:

Hashimoto's thyroiditis, an autoimmune disease that causes hypothyroidism
Graves' disease, an autoimmune disease that causes hyperthyroidism
Goiter
Thyroid nodule
Thyroid cancer

Hashimoto's is the leading cause of hypothyroidism, which is when the thyroid does not produce enough hormones to stimulate adequate metabolism. The condition is diagnosed by blood tests that measure TSH (thyroid stimulating hormone) and detect Hashimoto's antibodies. Symptoms of hypothyroidism include sluggish metabolism, inflammation, fatigue, weight gain, brittle hair, dry skin, and sensitivity to cold. To supplement impaired thyroid function, patients with Hashimoto's are typically prescribed synthetic thyroid hormone pills. Such patients may need to consume synthetic thyroids for the rest of their lives. Other treatments may include dietary changes, such as the avoidance of certain foods and medication to reduce inflammation. Frequent blood tests can inform patients if their TSH levels are within a normal range.

Grave's disease is most frequently the cause of hyperthyroidism, which is when the thyroid produces too many hormones, overstimulating one's metabolism. The condition is also diagnosed by blood tests that measure TSH and detect Grave's antibodies. Elevated heart rate, high blood pressure, sweating, bulging eyes, and weight loss are common symptoms of hyperthyroidism. Treatments for hyperthyroidism include oral radioactive iodine, which slowly shrinks the thyroid, slowing the production of excess thyroid hormone. Other anti-thyroid medications such as methimazole (Tapazole) also reduce the production of thyroid hormones. Blood pressure medications known as beta blockers can ease the increased heart rate caused by hyperthyroidism. If treatments are unsuccessful, patients may be prescribed a thyroidectomy, or surgery to remove part or all of the thyroid. Some patients with hyperthyroidism may require ophthalmological (eye medicine) treatments such as eye drops, prednisone, and surgery to reduce eye swelling and bulging.

Untreated thyroid disorders frequently lead to goiters or thyroid nodules. A thyroid goiter is a large lump that can be felt near the base of the neck and is made of swollen thyroid tissue or nodules (excess cell growth). Nodules and goiters are diagnosed by CT scan or ultrasound. Obstructive goiters can cause pain, coughing, and abnormal breathing. The most common cause of goiters is hyperthyroidism (overactive thyroid). However, Hashimoto's is also known to cause goiters or nodules. Goiters develop from continued damage from antibodies to the thyroid over time. At times, a goiter can result from a cancerous thyroid tumor. Most goiters and nodules are themselves benign.

The exact causes of thyroid cancer are unknown. People who develop thyroid cancer may or may not have another thyroid problem such as Hashimoto's or Grave's disease. Symptoms of thyroid cancer include a nodule that can be felt on the neck, hoarseness, difficulty swallowing, swelling in the neck, and neck and throat pain. CT and MRI scans are used to diagnose thyroid cancer. There are four types of thyroid cancer which vary in intensity. Thyroidectomy (thyroid removal surgery) is usually prescribed to treat thyroid cancer.

Thyroid problems are often long-term conditions that require some form of treatment. With regular monitoring and medication, many people with thyroid problems are able to lead active and fulfilling lives.

Prior to developing T1DM, I already managed multiple ADA-recognized disabilities that are 'invisible.' Having T1DM has presented profound challenges for me--as it doesn't play well with my other disabilities and routines. For example, some meds I take (of which there are no alternate options that work) inhibit appetite, a challenge to coordinate with T1DM. Because of this and other more complex challenges, I'm having to make some deep behavioral changes at a relatively late stage in life. I know I wouldn't be where I am today--succeeding in my ability to manage these conflicting disabilities and daring to thrive--if it weren't for Dr. Davies's expert medical expertise and exceptional and progmatic empathy.Dr. Davies is extraordinary in his ability to provide exceptional medical care for my T1DM and to actively listen with empathy to the diverse range of challenges I face. However, what I find especially remarkable is how he builds pragmatism and empathy into the T1DM learning I receive from him and his team. Not only does this approach help me grow my understanding of T1DM without becoming overwhelmed AND succeed in changing long-held behaviors, It's been critical in helping me develop my own pragmatic, empahty-centered structure around my T1DM care. To be clear: No other highly rated, expert provider I work with--excepting Dr. Davies and the team he's assembled--understands how T1DM can (and does) interact my other disabilities AND is highly skilled, empahty-centered, and successful at transferring the learning at the pace I need to comprehend and optimize my daily management of T1DM. I'm beyond grateful. Thank you.

Dr Davies exceeded my expectations of understanding my overall health needs way beyond his endocrinologist area of specialty. He coached and encouraged me to immediately begin an exercise routine to improve my strength, agility, and cardio. No other doctor I've ever seen has done that. He also heard and incorporated my financial challenges into the care plan, by differentiating between what was optional and what was required. With Dr Davies, there is no shame with being poor. I don't have a medical background, but everything he described regarding my diagnosis and treatment plan was easy to understand. He even drew pictures and a process flow clearly explaining the root cause. When he set the requirement of 3 liters a day of liquids, for my kidney and overall health, I balked. But he was persistent and explained half a dozen ways to make consuming an extra liter a day doable. I have adopted those suggestions and it's working. I've also adopted a daily exercise schedule and my overall health is improving. Finally, he even listed a task of getting financial aid, etc to address my insufficient healthcare plan. I've since qualified for WA state AppleCare and am reviewing coverage to determine how much of the treatment plan is covered, which is much better than my previous plan that covered nothing. Dr Davies is an A+ specialist.

Dr Davies is the best hands down. I wish I could have him for all my doctoring. He is above any doctor I have ever been to. He is caring which most doctors are but he goes the extra miles always. He looks at all things concerned with you health an has been overly helpful to me because his expertise is amazing. He always gets right back to you if you send him a message. He cares about you and your health. He looks at things in different ways than other doctors and it was amazing to me how he could figure things out with me in a short time. He is the best doctor ever! He is so honest and upfront with his patients. Does not make you feel like you are taking up to much time. I have never had a better doctor as Dr Matthew Davies. He jus a very dedicated thoughtful and compassionate doctor. He always goes above and beyond to help in all your health issues. And I like his way of thinking about issues and things in life. Very healthy, logical and supportive. He always goes above and beyond to work towards a healthy outcome for me. And I am not very difficult with a lot of issues where others would just stop and not dive into things. But he searches any issues and help you work on them. I so appreciate this doctor that I wish he could be my doctor for any and all medical issues. I travel from Puyallup to see him which I do not like driving to Seattle any more but I do it because I look so forward to seeing him each year. He is the best. I feel so blessed to have him as my doctor.

Dr. Davies seems to be an excellent clinician. He recognized my need for acute care when consulted by my primary physician and accepted me as a new patient immediately. His evaluation, diagnosis and treatment plan was swift and exactly what I needed at the time. His MyChart responses have always been quick and to the point. He has a critical eye on the numbers, as well as physical assessment. He offers critical opinions about personal lifestyle that may or may not fit the patient's idea of what is normal for them. Overall, he offered excellent guidance in critical phase, though eventually seemed somewhat annoyed by my sense that emotional stresses may be a factor (which he initially brought to my attention during diagnosis). After meeting with him in person, I prefer video interactions.

My experience with this office has been exceptional due to Dr. Davies and his assistant. He has been caring for me in regards to my finicky thyroid condition for 15 or so years now. He's always been an incredibly careful listener and communicator. I have confidence in his expertise--which he always thoughtfully combines with what I tell him about how I am feeling/symptoms so that we end up in the right place for me. Whether in the office, on zoom, or with follow up phone calls or emails, Dr. Davies is beyond prompt, always thoughtful and meticulous. Dr. Davies is a shining star as a medical professional and I am so very grateful to have him as my doctor.

Dr. Davies is TOP NOTCH! He is always concerned with my health and thyroid numbers, asks good questions, writes thing down, makes good suggestions. I can't think of anything he doesn't cover with me. He is also pleasant, polite, humorous and super smart about everything thyroid. I couldn't ask for a better doctor to manage my thyroid condition. I feel really lucky to have him. He made some suggestions to help me on my health journey and commented that I looked better from last year. I lost about 30 lbs since our 2024 visit and am continuing on the road to better health.

Dr Davies is one of the best doctors I have ever had. As an engineer, I tend to do a lot of research by myself and ask a lot of questions. Dr Davies always takes the time to answer my questions and is very patient. He truly cares about his patients. Recommended Dr Davies to two other friends, and they also feel that Dr Davies is a great doctor. I appreciate his indepth knowledge in biochemistry and his ability to problem-solve and reason, rather than regurgitate something memorized from a medical textbook. I hope Swedish recognizes what a great doctos he is.

Dr. Davies has been overseeing an adrenal gland and hormonal condition caused by a non-malignant growth on the gland for several years. During that time I have seen him at least once a year where we checked all the lab results for changes and considered further treatment. I completely trust his judgment and guidance as we keep this condition under surveillance and I appreciate his cautious approach to deciding on treatment. I also find his good humor to be reassuring as I am always a nervous patient. Glad to have had him working on my behalf.

Dr Davies has been my endocrinologist for almost 20 years. He's treated me through multiple problems, issues, and serious crises, with creativity and total commitment. I am healthier than I have ever been, and I have a very much more optimistic view of my life expectancy due to the work we have done together. He and I have worked as partners; the results have been wildly successful. He isn't a cheerleader, but it turns out that I really don't need one. I need a doc who has faith in me and my strength.
