Autoimmune Diseases

An autoimmune disorder happens when the immune system mistakenly attacks the tissues of its own body, causing symptoms of illness. There are more than 80 different types of autoimmune disorders. While some are very rare, others are fairly common. Combined, autoimmune disorders are one of the leading causes of death and disability in the United States, affecting approximately 24 million people.

A properly working immune system identifies foreign substances in the body that might cause illness, such as bacteria and viruses. The immune system then creates antibodies which attack the foreign substances, neutralizing them and keeping the body safe. In people with autoimmune disorders, something goes wrong with this process. For reasons that are not fully understand, the immune system creates antibodies to attack the patient's own tissues.

Symptoms of an autoimmune disorder depend on which tissue is being attacked by the immune system, but common symptoms of autoimmune disease include fever, fatigue, and a general feeling of just not being well. Autoimmune disorders are more common in women than in men, and they may run in families. Autoimmune disorders can affect various parts of the body such as blood vessels, connective tissue, endocrine glands, joints, muscles, red blood cells, skin, and many others.

It is common to have more than one autoimmune disorder at a time. Most are chronic, or life-long illnesses, although they may come and go in flares. Treatment for autoimmune disorders depends on which part of the body is being attacked. For example:

  • A type 1 diabetic whose pancreas has been damaged will need insulin.
  • A person with Hashimoto's whose thyroid has been damaged will need replacement thyroid hormones.
  • Someone with Sjogren's syndrome will need eye drops and mouth rinses to replace tears and saliva.

Many autoimmune disorders of all kinds are treated with immune-suppressing medications, such as corticosteroids (e.g. prednisone) to reduce the effect of the immune system.

Headache

Headaches are dull or sharp pains that occur in regions of the head and face. Headaches occur in many different forms and vary in location, severity, and duration. They are not necessarily a sign of an underlying illness and often resolve on their own. However, headaches can present significant day-to-day discomfort. The most common forms of headaches include:

  • Migraines: one of the most prevalent types of headache, migraines typically last from two hours to three days, causing a throbbing pain on one side of the head. Nausea, sensitivity to bright lights, fatigue, and blurred vision all may accompany migraines. While migraines have no known cure, medications and lifestyle modifications may ease symptoms. Avoiding behaviors that may trigger migraines such as lack of sleep or loud, repetitive noises can improve headaches.
  • Tension headaches: also known as stress headaches, these headaches are characterized by pain in the upper top part of the head and behind the eyes. Tension headaches are common and have no exact cause; however, certain practices may increase the likelihood of developing them. For example, wearing one's hair up in a tight hairstyle or wearing a restrictive hat can put pressure on the scalp, leading to a tension headache. Other causes of tension headaches may include alcohol or nicotine use, caffeine, teeth grinding, stress, overexertion, and eye strain. Some people find posture improvement, alternating sleeping positions, and neck exercise can alleviate tension headaches. Over-the-counter pain relievers such as ibuprofen (Advil, Motren) and naproxen sodium (Aleve) may also reduce tension-headache pain.
  • Sinus headaches: this type of headache causes pain in the sinuses, but may progress to pain and pressure in the eyes, forehead, and cheeks. These headaches are often accompanied by nasal congestion, fatigue, and ache in the upper teeth and may or may not be caused by sinus infections or sinusitis (sinus inflammation). Some sinus headaches are allergic headaches, which arise from allergic triggers such as seasonal allergies (pollen allergies) or hay fever, causing sinus pressure and facial pain. Other symptoms of allergic headaches include nasal congestion, irritated or watery eyes, runny nose, and sneezing. Allergic headaches may be treated with antihistamine medications such as cetirizine (Zyrtec). Allergic headaches are common during pollen season, which is usually around springtime.
  • Cluster headaches: these headaches occur suddenly and cause stabbing, sharp pain from one eye down to the one side of the neck. They are often referred to as the most aggravating and painful type of headaches. Other symptoms of cluster headaches include nasal congestion, droopy eyelid, and enlarged pupil on one side of the face. In the past, it was widely believed that cluster headaches were caused by a bevy or "cluster" of multiple types of headaches, such as a combination of an allergic headache and a tension headache. Medical experts now recognize that cluster headaches are a separate and unique form of headache. Cluster headaches last for around twenty minutes to two hours. They may recur throughout the period of a day or several weeks, but usually dissipate within a month. Treatment for cluster headaches includes over-the-counter pain medications such as ibuprofen (Advil, Motren) and naproxen sodium (Aleve). Prescription Prednisone can help with swelling and Verapamil may reduce pain by relaxing the blood vessels.

Living with headaches is challenging; headaches can prevent people from fully enjoying life. Medical treatment and care in avoiding certain triggers can help those with headaches begin to feel normal once again.

Stroke

A stroke is a medical emergency that occurs when a blood vessel supplying oxygen and nutrients to the brain is ruptured or blocked. The brain cannot function without a steady supply of oxygen and nutrients, so when the blood supply to the brain is interrupted, even for a brief moment, brain cells begin to die. When a sufficient number of brain cells die, the brain itself can no longer function, meaning that strokes are very dangerous. Strokes can result in impaired movement, speech, cognitive ability, the impairment of important physical functions, and even death.

Anyone can have a stroke, regardless of age and health. Strokes are known to happen at random. However, certain conditions and behaviors can increase one's risk of stroke over time.

Risk factors for stroke include:

  • Heart disease
  • Older age
  • Smoking
  • Diabetes
  • High blood pressure
  • High blood cholesterol and lipids

There are two main types of stroke: ischemic stroke and hemorrhagic stroke. Ischemic stroke occurs more commonly and is when a blood vessel in the brain becomes blocked. Blockages are caused by blood clots and built-up plaque (deposits of fat and cholesterol), which leads to atherosclerosis, a condition where the blood vessels narrow and harden. The resultant restricted blood flow may lead to an ischemic stroke by blocking essential oxygen to the brain, causing the heart to exert more effort to pump blood.

Hemorrhagic stroke occurs when a blood vessel in or around the brain ruptures. The burst vessel leaks blood into surrounding brain tissue, causing irreparable brain damage.

Hemorrhagic strokes are less common and may occur from head injuries, brain aneurysms, brain tumors, and bleeding disorders, amongst other conditions.

Both ischemic and hemorrhagic strokes require immediate medical treatment. Medical intervention can prevent severe and life-threatening brain damage. Signs of stroke include:

-Disorientation or confusion

-Difficulty speaking

-Difficulty walking

-Impaired vision

-Weakness in the face, legs, or arms

-Severe headaches

When someone suspected of having a stroke reaches the hospital, the patient will usually receive a CT or other scan that provides an image of the brain. The scan results can reveal clotted or leaking blood in the brain. Physical exams, neurological exams, and blood tests may also be used to check for blood clotting and sugar. Treatment begins generally immediately if a stroke is detected.

Medical treatment for ischemic strokes includes a thrombolytic medicine which breaks up and disperses blood clots. This medicine can greatly improve stroke recovery and long-term health. However, the thrombolytic medicine must be administered as soon as possible to achieve these results. Other treatments for ischemic strokes are blood thinners and thrombectomy, a surgical procedure to remove a blood clot in the brain. Thrombectomy is performed by inserting a catheter (thin needle) into the artery to reach the blood clot and mechanically remove it. With the clot removed, normal blood flow to the brain resumes. Thrombectomy is typically performed at most six hours after a patient shows signs of a stroke.

Hemorrhagic strokes are treated with blood-saving medication and surgery. For a hemorrhagic stroke caused by the rupture of an aneurysm (blood vessel bulges), surgery to stem the bleeding aneurysm and vessel may be used. One surgical technique is stent-assisted coiling, which adds a stent (small wire-meshed tube) into the blood vessel to block the leaking opening of the aneurysm. A non-surgical procedure is endovascular coiling, where a catheter (thin, hollow needle) places a platinum wire coil at the spot of the aneurysm. The coil blocks bleeding and prevents the aneurysm from continuing to leak blood. These procedures are usually combined with blood-thickening medications. Blood transfusions may be administered if substantial blood loss has occurred.

Since stroke patients may have lost some essential functions while the blood flow to their brains was obstructed, both ischemic stroke and hemorrhage stroke patients may receive post-stroke rehabilitation to rebuild their physical and mental abilities. Some restorative treatments include occupational, physical, and speech therapy. Patients may also receive care from neurologists, who can assist patients in recovering certain brain functions and cognitive abilities, and rehabilitation psychologists, who may help patients with their emotional, behavioral, and cognitive recovery. Some stroke patients require lengthy rehabilitation and may never fully regain function, while others may recover more quickly. While the fastest recovery improvements are usually seen in the first three or four months, stroke rehabilitation can last for years.

If someone begins to show signs of stroke, contact emergency services and call 9-1-1 immediately since blood flow must be restored as soon as possible to help avoid significant brain damage. If an ambulance takes the person to the hospital, paramedics can initiate stroke treatment as soon as the patient enters the ambulance, allowing for more prompt medical intervention and care.

Go anywhere else. I saw Dr. Kesting after my stroke. He was rough with me despite my osteoarthritis. When I told him I was in pain he kept going. He ignored my concerns about feeling constantly light-headed and failed to do little to any neurological testing upon follow-up. I know there aren't many neurologists in Olympia but paying someone to mistreat you and provide inadequate care isn't worth your time.

Because of the fallout from Covid, like all medical personnel and facilities, it was difficult to get an appointment in a timely manner. Having said that, Dr. Kesting is/was the most prepared physician I have ever seen in 72 years. When he came into the exam room he was completely prepared having reviewed and taken hand written notes for himself. He was merely courteous but personable, professional, curious, very thorough and extremely patient with the concerns of both me and my wife. He carefully and completely answered all of our questions and concerns and we could not have hoped to have a better physician address my recently diagnose Myasthenia Gravis issues. Were all the medical profession as good as Dr. Kesting!

Dr Kesting excellent. A Dr.who listened, Showed concern, ask questions about condition & explained what he was looking for and any deficits I have. I was there as new patient. Was so pleasant having a Dr show Respect to me. He is 1st Dr in 30yrs that gave real Dr/Pt concern. Highly Recommend. Want to add the front desk staff were very friendly, pleasant, informative, knowledgeable. A warm welcome. The MAs were also very kind, took their time checking me in, asking pertinent questions, knowledgeable friendly. Excellent check in from all. Thank them for super service. I'm a nurse and know gd care when I see it.

I am so glad that I found Dr. Kesting! I got blood work done the same day and the nurse called and left me a voicemail the next day letting me know what I needed to do. He also ordered imaging and I got a call from the hospital the next day! I live in Mason county and the providers never call and follow-up that fast and you don't hear from imaging to schedule an appointment for at least a month! I am so glad I went to Dr. Kesting and will be seeing him for all of my neurology needs!

Dr. Kesting is the most thoroughly prepared physician I have dealt with in a practice in the last 18 years. He spends his time consulting his hand written notes (from before he enters the room) then listening, analyzing and explaining everything completely. Unlike most Doctors, N.P.s, P.A.s, he does not spend 95% of his time in the room with me on the computer. How refreshing and confidence building for a patient. I would like to clone him into every line of medicine.

I have been a patient of Dr. Kesting for past balance issues but hadn't visited him for about 5 years. At this visit we reviewed my past condition as well as my current condition. Dr. Kesting completed a thorough review and we discussed my current situation at length. He advised me to pursue physical therapy and have a followup visit in six months. Dr. Kesting has always provided excellent care, listened to all my concerns and is always very thorough.

This was my first experience with a Neurologist. I had many concerns and questions about the reason for the referral which had to do with my having seizures in January 2024.. Dr. Kesting was very patient in addressisng my concerns and explaining what would happen next. He was very patient with me especially when I had trouble doing some of the exercises. I felt very comfortable talking with him. It was a good experience.

Speaking strictly about this particular provider and the staff there, they are genuinely professional and courteous without being inauthentic or too talkative. I appreciate it because I see a lot of Doctors regularly and sometimes people are too friendly and talkative when all I really want is basic courtesy and professionalism. This office is very good at that basic but all too rare skill.

The doctor was easy to relate to and was obviously listening and observing me. The office staff was cheerful and kind. I got a glass of water given to me on request. The staff helped me check out the windows to see if my car was in the sunlight yet. My little dog was In my car and I wanted to be sure she was not getting overheated and in danger. The feeling was warm and familial.

First visit. Went well, not clear why headache and memory issues are to be discussed further next visit, discussed cat scan of brain but no indication when if this will be ordered. Overall Dr was nice, just wish steps for what comes next or what to expect was explained better. Dr was nicer than I expected. Office staff friendly and. Helpful
