Headache

Headaches are dull or sharp pains that occur in regions of the head and face. Headaches occur in many different forms and vary in location, severity, and duration. They are not necessarily a sign of an underlying illness and often resolve on their own. However, headaches can present significant day-to-day discomfort. The most common forms of headaches include:

  • Migraines: one of the most prevalent types of headache, migraines typically last from two hours to three days, causing a throbbing pain on one side of the head. Nausea, sensitivity to bright lights, fatigue, and blurred vision all may accompany migraines. While migraines have no known cure, medications and lifestyle modifications may ease symptoms. Avoiding behaviors that may trigger migraines such as lack of sleep or loud, repetitive noises can improve headaches.
  • Tension headaches: also known as stress headaches, these headaches are characterized by pain in the upper top part of the head and behind the eyes. Tension headaches are common and have no exact cause; however, certain practices may increase the likelihood of developing them. For example, wearing one's hair up in a tight hairstyle or wearing a restrictive hat can put pressure on the scalp, leading to a tension headache. Other causes of tension headaches may include alcohol or nicotine use, caffeine, teeth grinding, stress, overexertion, and eye strain. Some people find posture improvement, alternating sleeping positions, and neck exercise can alleviate tension headaches. Over-the-counter pain relievers such as ibuprofen (Advil, Motren) and naproxen sodium (Aleve) may also reduce tension-headache pain.
  • Sinus headaches: this type of headache causes pain in the sinuses, but may progress to pain and pressure in the eyes, forehead, and cheeks. These headaches are often accompanied by nasal congestion, fatigue, and ache in the upper teeth and may or may not be caused by sinus infections or sinusitis (sinus inflammation). Some sinus headaches are allergic headaches, which arise from allergic triggers such as seasonal allergies (pollen allergies) or hay fever, causing sinus pressure and facial pain. Other symptoms of allergic headaches include nasal congestion, irritated or watery eyes, runny nose, and sneezing. Allergic headaches may be treated with antihistamine medications such as cetirizine (Zyrtec). Allergic headaches are common during pollen season, which is usually around springtime.
  • Cluster headaches: these headaches occur suddenly and cause stabbing, sharp pain from one eye down to the one side of the neck. They are often referred to as the most aggravating and painful type of headaches. Other symptoms of cluster headaches include nasal congestion, droopy eyelid, and enlarged pupil on one side of the face. In the past, it was widely believed that cluster headaches were caused by a bevy or "cluster" of multiple types of headaches, such as a combination of an allergic headache and a tension headache. Medical experts now recognize that cluster headaches are a separate and unique form of headache. Cluster headaches last for around twenty minutes to two hours. They may recur throughout the period of a day or several weeks, but usually dissipate within a month. Treatment for cluster headaches includes over-the-counter pain medications such as ibuprofen (Advil, Motren) and naproxen sodium (Aleve). Prescription Prednisone can help with swelling and Verapamil may reduce pain by relaxing the blood vessels.

Living with headaches is challenging; headaches can prevent people from fully enjoying life. Medical treatment and care in avoiding certain triggers can help those with headaches begin to feel normal once again.

Mononucleosis (Mono)

Mononucleosis, frequently called ''mono,'' is a viral infection that causes fatigue, fever, swollen lymph glands, and sore throat. The Epstein-Barr virus (EBV), spread through saliva, is the primary virus that leads to mono. Mono is most common in teenagers and young adults, where EBV is often transmitted through kissing or sharing drinks. For this reason, mono is colloquially referred to as ''the kissing disease.'' Mononucleosis usually resolves on its own; however, recovery from mono can take from several days to several weeks. Some young adults may continue to experience extreme fatigue from mono for months, often requiring a semester home from school.

Mononucleosis happens in three stages:

During the initial stage, those with mono may experience few symptoms as EBV replicates in the body. Symptoms such as headaches and body aches are often misattributed to allergies or stress. People with mono may continue performing their daily activities and attending school, leading to increased transmission of the virus to others. The initial stage lasts for about five to fourteen days.

During the acute stage, mono symptoms intensify and may include the enlargement of the spleen, a sore throat, fatigue, liver issues, and skin rashes. The mono infection is at its worst in this stage.

Lastly, the convalescent stage is when recovery begins. This period of healing can last from two to six months.

Mono can be detected by a physical exam; physicians can diagnose mono by checking for inflamed lymph nodes, tonsils, or spleens. Blood antibody tests may also help diagnose mono by revealing antibodies in the blood that were formed as a response to the Epstein-Barr virus. Elevated levels of white blood cells can also suggest that a patient has mono.

There are no medications specifically for mono or the Epstein-Barr virus, and patients are advised to rest and avoid strenuous exercise. Drinking plenty of fluids, eating a healthy diet, and taking anti-inflammatory ibuprofen (motrin) may also help people who are afflicted with mono feel better.

People with mono should take precautions to avoid spreading the condition to others. They can prevent infection by not sharing glasses, food, and utensils with others until they fully recover.

Thyroid Problems

Thyroid problems are a group of conditions that alter hormone regulation and metabolism. The thyroid is a small, butterfly-shaped gland located in the lower neck, just below the Adam's apple (laryngeal prominence) and larynx (voice box). Despite its small size, the thyroid has an outsized effect on the body by regulating hormones that control metabolism. Metabolism is the body's process of converting consumed food into fuel to power the entire body. Heart rate, weight, cholesterol, body temperature, and even physical growth and development are all affected by the metabolism.

When the thyroid malfunctions, the body is unable to properly process and allocate energy. Thyroid problems, such as hyperthyroidism (overactive thyroid) and hypothyroidism (underactive thyroid), cause irregular metabolism. In some cases, the body produces antibodies that attack the thyroid cells for unknown reasons. The damaged thyroid responds by releasing either too little or too much T3 and T4 thyroid hormones. Thyroid malfunctions may also arise from genetic causes or iodine deficiency. Thyroid problems may include:

  • Hashimoto's thyroiditis, an autoimmune disease that causes hypothyroidism
  • Graves' disease, an autoimmune disease that causes hyperthyroidism
  • Goiter
  • Thyroid nodule
  • Thyroid cancer

Hashimoto's is the leading cause of hypothyroidism, which is when the thyroid does not produce enough hormones to stimulate adequate metabolism. The condition is diagnosed by blood tests that measure TSH (thyroid stimulating hormone) and detect Hashimoto's antibodies. Symptoms of hypothyroidism include sluggish metabolism, inflammation, fatigue, weight gain, brittle hair, dry skin, and sensitivity to cold. To supplement impaired thyroid function, patients with Hashimoto's are typically prescribed synthetic thyroid hormone pills. Such patients may need to consume synthetic thyroids for the rest of their lives. Other treatments may include dietary changes, such as the avoidance of certain foods and medication to reduce inflammation. Frequent blood tests can inform patients if their TSH levels are within a normal range.

Grave's disease is most frequently the cause of hyperthyroidism, which is when the thyroid produces too many hormones, overstimulating one's metabolism. The condition is also diagnosed by blood tests that measure TSH and detect Grave's antibodies. Elevated heart rate, high blood pressure, sweating, bulging eyes, and weight loss are common symptoms of hyperthyroidism. Treatments for hyperthyroidism include oral radioactive iodine, which slowly shrinks the thyroid, slowing the production of excess thyroid hormone. Other anti-thyroid medications such as methimazole (Tapazole) also reduce the production of thyroid hormones. Blood pressure medications known as beta blockers can ease the increased heart rate caused by hyperthyroidism. If treatments are unsuccessful, patients may be prescribed a thyroidectomy, or surgery to remove part or all of the thyroid. Some patients with hyperthyroidism may require ophthalmological (eye medicine) treatments such as eye drops, prednisone, and surgery to reduce eye swelling and bulging.

Untreated thyroid disorders frequently lead to goiters or thyroid nodules. A thyroid goiter is a large lump that can be felt near the base of the neck and is made of swollen thyroid tissue or nodules (excess cell growth). Nodules and goiters are diagnosed by CT scan or ultrasound. Obstructive goiters can cause pain, coughing, and abnormal breathing. The most common cause of goiters is hyperthyroidism (overactive thyroid). However, Hashimoto's is also known to cause goiters or nodules. Goiters develop from continued damage from antibodies to the thyroid over time. At times, a goiter can result from a cancerous thyroid tumor. Most goiters and nodules are themselves benign.

The exact causes of thyroid cancer are unknown. People who develop thyroid cancer may or may not have another thyroid problem such as Hashimoto's or Grave's disease. Symptoms of thyroid cancer include a nodule that can be felt on the neck, hoarseness, difficulty swallowing, swelling in the neck, and neck and throat pain. CT and MRI scans are used to diagnose thyroid cancer. There are four types of thyroid cancer which vary in intensity. Thyroidectomy (thyroid removal surgery) is usually prescribed to treat thyroid cancer.

Thyroid problems are often long-term conditions that require some form of treatment. With regular monitoring and medication, many people with thyroid problems are able to lead active and fulfilling lives.

Dr. Kim is very kind, understanding, respectful and focused on the patient. He gave me simple tips on how to improve my health. He was meticulous about getting a report on a procedure I had that had not been transmitted to his office as I had requested. He is thorough. He reviewed my lab results with me and gave me a copy. He went over the tests that I must have periodically (mammogram, colonoscopy) and made sure I understood when I was due for another test. One time, when reviewing a pre-surgery order for tests from my surgeon's office, he noticed an omission and ordered the necessary test on his own. I feel very calm and comfortable when I am with him, because I trust him and I know that I am getting the best care. The support staff is excellent - very efficient, pleasant and helpful. A staff member got me a prescription refill written by another doctor - the same day I called! I appreciate greatly the decision of Providence Health Care to establish a senior care clinic, where my husband and I can get such excellent care.

I actually look forward to medical appointments! Dr. Kim is not only cheerful and very respectful, but knows my medical needs and keeps up with what testing is available, recommending all he thinks will be helpful for me. We had long discussions this time about a variety of topics. He listened carefully, gave great feedback, and scheduled tests to cover my concerns. I like being treated with respect and listened to as if my words are Important. Never has he rushed through an appointment - he gives me the time I need to resolve my Issues. I always leave his office feeling I matter as a person and my concerns have been listened to and resolved to my satisfaction. I often recommend him to others and feel I am getting the best care possible from him and his staff.

Dr. Kim is a very caring doctor. He has a wonderful bedside manner, listens to my concerns and makes suggestions after being thoroughly informed through information he gathers from me and with his knowledge of medicine I truly appreciate him and I have often recommended coworkers to him I know he's not taking more patient, but when he does, I know that my coworkers would love to go in and see him. I would personally like to thank Dr. Kim for all his kindness and understanding to myself and my parents as they also see him as well.

Dr Kim is a very caring person which enhances his medical skills. He respects that I know what my body Is telling me, when I describe purpose of visit. Generous with details when talking about my needs and particular treatments or medicines. As a very senior person, I never have walked away from DR Kim feeling Ignored or dismissed. All my visits let me know that I am old but doing and feeling great. His parting handshake connects us and shares our mutual respects. PROVIDENCE and we citizens are so fortunate to have him.

Providence care is excellent. Unfortunately the facility is impossible to navigate. I always get lost. Dr Kim is an excellent physician, perhaps the best I've ever had. I am so grateful to be working with him. I was trained as a teach of English to non-English speakers and I'd say he needs to work with a speech therapist to improve his English. He can be very difficult to understand. Thank you for the excellent care I'm getting.

Dr Kim is the best doctor that I have ever had. He is always very attentive and an active listener. On this occasion I was updating him about my bladder condition. He recommended the best surgeon for my condition, and the next day sent a referral to that doctor's office enabling me to schedule an early appointment. Dr Kim is a very good doctor. All his staff agree. I am fortunate to have him managing my care!

I live 50 miles from Dr. Kim's office. I have had times I needed to see him the same day. I've either been very lucky or they have accommodated my need that day. Dr. Kim is my favorite Dr. Of all Dr's. I have. He listens and answers ALL of my concerns. I live in Waslla, and he helps to get needed referrals for testing near my area so I don't have to travel to Anchorage. I recommend Dr. Kim very much.

Dr. Kim had been my GP for almost two decades. I couldn't ask for a more kind and compassionate physician. He's always available to listen and any answer questions I may have. And I usually do. Honestly, if Dr. Kim ever retires, I'll certainly be at a loss as to who to see. I appreciate Dr. Kim & his staff and hope I never have to make that choice. Thank you Dr. Kim.

Dr Kim is the most caring doctor I have ever seen. He is always concerned for my safety and wellbeing. De Kim and his office have given me excellent care since I first started seeing him about 10 years ago. His staff has been polite and helpful, despite it getting for me to remember thinks. I trust them. I have had experienced with Alaska Doctors to the extend I considered suing them. I can see that happening with Providence at all.
