Anemia

Anemia is the most common blood disorder. It occurs when the body does not have enough red blood cells to carry oxygen to the tissues. Blood loss, insufficient red blood cell production, and red blood cell destruction are the three main causes of anemia. These issues may arise from such conditions as iron deficiency, heavy periods, ulcers, kidney disease, as well as some forms of cancer.

An individual with anemia will have the following symptoms: shortness of breath, weakness, dizziness, cold hands or feet, paleness, and irritability. Anemia is diagnosed when blood tests show low counts for hemoglobin, which is the protein responsible for carrying oxygen to different tissues throughout the body.

Treatments for anemia are dependent on the kind of anemia an individual has. For example, iron supplementation may correct iron deficiency anemia. For anemia caused by chronic kidney failure, chemotherapy treatments, and HIV/AIDS, a group of medications called erythropoiesis-stimulating agents may be prescribed.

Dialysis

Dialysis is a procedure that replicates renal (kidney) function by filtering patients' blood. When kidneys do not work properly, waste can accumulate in the blood and unbalanced chemicals can impair the body's critical functions. In order to stay healthy, a person without proper kidney function must receive dialysis. There are two forms of dialysis treatment: hemodialysis and peritoneal dialysis. Both forms of dialysis are recurring treatments that in many cases last throughout the lifetime of the affected patient. Patients who briefly lose renal function may slowly reduce the frequency of dialysis sessions until their kidneys recover. However, most patients who start on dialysis remain on dialysis for the rest of their lives or until they receive a kidney transplant.

A number of conditions can worsen renal function and lead to end-stage or acute kidney failure. When kidneys fail (i.e. nearly 90 percent of their function is lost), dialysis is typically prescribed. Conditions that cause chronic kidney failure (eventually requiring dialysis) include:

Type 1 and 2 Diabetes
Pyelonephritis, recurring kidney infection
Glomerulonephritis, inflammation of the kidney's filtering parts
Polycystic kidney disease
Interstitial nephritis, inflammation of the kidney's tubules
High blood pressure
Vesicoureteral reflux, improper bladder emptying

For hemodialysis (the more common form of dialysis), a patient will visit a hospital or clinic and be connected to a dialysis machine by a needle attached to a tube that draws blood from the arm. The drawn blood is transferred to the dialysis machine, where it is filtered and separated until clean. Waste products from the blood pass into a fluid called dialysate, which is pumped out of the machine into a waste receptacle. The machine also measures and helps ensure the blood has the appropriate level of fluid, electrolytes, and pH. A tube delivers the cleaned blood back into the patient's body. A dialysis session like this will typically last for three to four hours, with a patient undergoing dialysis around three times per week.

A second method of dialysis is known as peritoneal dialysis. Unlike hemodialysis, peritoneal dialysis can be performed at home by oneself after an initial surgery. To start, a physician (generally a surgeon) will make a small incision in the lower abdomen and insert and surgically attach a catheter (thin tube). At home, the patient connects a pump to the abdomen catheter, delivering dialysate (dialysis fluid) from a bag hanging on a wheeled stand. This fluid enters the peritoneal cavity (greater abdomen area containing the stomach, liver, and intestines) and collects waste through osmosis, where waste in the blood moves across a membrane and into the dialysate. This process continues for several hours until the fluid concentration is equal between the blood and dialysate, at which point the fluid can be drained. The fluid can then be passed through a machine called a cycler, which removes waste and allows for the dialysate to be reused. This process is repeated about four times per day. Dialysis patients must limit the amount of fluid they consume prior to receiving dialysis and should also avoid eating salty foods. The cycler cannot filter more than a certain amount of waste products from the blood.

Kidneys are important organs, and dialysis treatments are vital for those with renal dysfunction. For many, dialysis is not so much a medical procedure but a part of their normal life.

Kidney Stones

Kidney stones are hard deposits that form in the kidneys, made up of minerals that are normally present in urine. They can vary in size, from as small as a grain of sand to as large as a nickel, occasionally even larger. Sometimes they lodge in the kidney, and sometimes they break free and make their way out through the urinary tract, which can be extremely painful.

Kidney stones can be smooth or jagged and are yellow to brown in color. They are mostly comprised of the minerals calcium, oxalate, and phosphorus. Examining the stones to see what they are made of can show what caused the stone to be formed in the first place. For example, a stone made of mostly calcium, which is the most common type, can happen any time the urine becomes too concentrated due to dehydration or a blockage in the kidney. A uric acid stone forms when acid levels in the urine get too high, usually due to excessive consumption of animal protein such as meat and fish. A struvite stone is a sign of certain infections, and a cystine stone can be due to a genetic disorder that raises the risk of kidney stones.

The most common symptom of kidney stones is pain, either in the back or lower abdomen, or severe pain when urinating. There may also be blood in the urine. Treatment for kidney stones depends on how large the stone is. Very small stones can pass out of the body on their own, and they do not require treatment other than drinking adequate water and taking pain killers. Larger stones need to be broken apart and removed. The main treatment options are:

Shock wave lithotripsy, which uses sound wave vibrations to break apart the stone
Percutaneous nephrolithotomy, or the use of a very tiny tool (like a wire inserted through the back) to break apart and remove the stone
Ureteroscopy, a thin tube inserted through the urethra and bladder to the stone, where tiny tools can grasp the stone and remove

People who have had one kidney stone are at risk of developing another. To reduce this risk, patients are given instructions specific to the type of stone they developed. Generally the instructions will include drinking more water to dilute the urine, but it may also involve lowering sodium intake or eating less meat.

Well as far as my care provider himself there's no complaints and I'm using this word very specific as a medical doctor he's only made me feel good no matter if there's some bad news about my health he always finds kind words to express his concern and the outcome and the results he is just one of the best doctors I could never say anything bad about Dr Arif my mother cared about him and I have the same loving respect for him as a professional medical doctor it's beyond his calling he chose this through intervention or something because he is the perfect doctor I'm so I could never complain it would be a sin for me to complain about Dr Arif he's perfect in my mind and my eyes and all he does for me as a medical professional and I'm sorry that I mixed personal feelings than this but that's just how I feel and yes I rate him the top three doctors in my life that I've had to visit I mean 20 years plus tells you that he's doing everything right as far as his medical practice you know what he's honored to do for us so he's fulfilling his part and I respect him to the fullest and I'm just blessed to have him as one of my medical professionals that sees and doctors over me you know medically speaking I couldn't have it any better wow I'm having a an epiphany or a Deja Vu moment or I lived this before LOL thank you again for your concern of how we feel and see and think of our medical professionals but you have my opinion my most sincere opinion and thoughts of doctor Ariff... Dr. Arif is a wonderful doctor. His care and compassion for his patients. He genuinely cares and wants you to have your best life possible.I was in pretty rough shape kidney wise when I very first met Dr Arif. It got to the point of looking at dialysis for the rest of my life. Wasn't really a fan of that option so he set me on the right path to improving my kidney health and raising my numbers. My Gfr was down to 30. It's been a struggle but I finally made it. At this appointment I got really good news number wise across the board. My Gfr was 77!! Love never have been that high since before they told me that I needed to speak with a nephrologist. I prayed that I would be able to find a competent and compassionate Dr in the Tri-city area and not have to drive for hours to even for care. I did some research and saw Dr. Fareed Arif Nephrologist. I made an appointment which was within a couple weeks. This was back in the early 2007. I wish Providence would hire more Drs like Fareed Arif here at Kadlec Regional medical center in Washington State..

Dr Arif Fareed has been a tremendous help and source of information that has helped me deal with my CKD better. Not only has he managed my CKD but also caught a very important concern relating to another illness I have that was identified because of his attention to my blood work and sent me to see an Oncologist and am now able to be watched for any developments that could become life threatening. I owe him a great deal for this. He talks to me about all the panels in my lab work and advised me on things to improve my health. He is courteous and frank. I am lucky to have him leading my team of experts.

Dr Arif is a gift from God. He is always respectful and caring. He always asks about my family and is kind and wonderful. He listens carefully and helps me understand what I need to do. He's never hurried or interruptive. He's my doc for life's curveballs. Thank you Dr Arif. His office assistant and front desk staff are also top notch. My Sister in law with the same name and I were both scheduled at the same time different docs was a little confused at first but checked carefully and did her due diligence to make sure we were who we were. It was awesome.

Dr. Arif is my kidney Doctor, I see him 3 times a year and I always have a blood test one week before my appointment, so he can see if I have been taking my pills and eating right. Arif goes over all my blood numbers and tells him how I am doing. Also shows me on the computer screen what he is talking about. It Is very Interesting to me. Then he listens to my heart and my lungs and touches my ankles. To make sure I am doing OK. I am glad I go to Dr Arif for my kidney problems, I always leave to go home with a good feeling from the visit with him.

Dr Arif is very caring and always explains what is going on with my health issues and if I have ANY questions he is so great to listen to my concerns even if it concerns another Dr. I have an aneurysm on my heart that is of great concern to me but isn't really one of Dr Arifs issues with me but he listens to me because of my blood pressure. I love how he doesn't separate my Kidney issues from any other issue I may have. He listens. That makes a terrific Dr. Not many of them left and believe me, at my age I've known a lot of Drs. He's top notch.

I have always had a very good experience with Dr Arif. He has the best personality and is friendly and always has a smile. You'd never know if he was having a bad day. I love Dr Arif and trust him completely with my care and to know how to treat my situation. I see him every 3months followed by my lab. I hope and pray I never have to have a different Dr take over my care. I just lost my primary caregiver after many many years with her. It's not easy to start over especially at my age. Trust takes time. Do NOT lose Dr Arif please .

Dr. Arif was my mother's nephrologist and I admired then his bedside manner in which I wanted to make sure I would also see him for my own health concerns. He has great customer care and doesn't make you feel like just another patient. I still have yet to hear back regarding my bloodwork but I am assuming I will be informed at the next scheduled visit. Staff is absolutely amazing, from front office to medical assistants. I sincerely wish all Kadlec offices had the exact same customer service as this office.

Dr Arif Fareed is an excellent Nephrologist who is always pleasant and uplifting. Thanks to his attention to details he has possibly saved my life when he saw an anomaly in one or more panels when going over the results of my blood work and scheduled an appointment with an Oncologist which found that I did in fact have a blood disorder that prompted a regular checkup in order to catch any problems as soon as posible. Dr Arif is very thorough in his care of his patients. I will be forever grateful for this.

All good experiences. Dr. Arif listens, he is proactive, he is careful about preserving kidney function to the extent possible when advising about tests needed to monitor my kidneys, as well as tests other doctors would like to run to diagnose non-kidney issues, or prescriptions they want to prescribe when there is a potential effect on my kidneys. The decision is always up to me, but he is good to offer all the pros and cons so my decision is well-informed..
