Autoimmune Diseases

An autoimmune disorder happens when the immune system mistakenly attacks the tissues of its own body, causing symptoms of illness. There are more than 80 different types of autoimmune disorders. While some are very rare, others are fairly common. Combined, autoimmune disorders are one of the leading causes of death and disability in the United States, affecting approximately 24 million people.

A properly working immune system identifies foreign substances in the body that might cause illness, such as bacteria and viruses. The immune system then creates antibodies which attack the foreign substances, neutralizing them and keeping the body safe. In people with autoimmune disorders, something goes wrong with this process. For reasons that are not fully understand, the immune system creates antibodies to attack the patient's own tissues.

Symptoms of an autoimmune disorder depend on which tissue is being attacked by the immune system, but common symptoms of autoimmune disease include fever, fatigue, and a general feeling of just not being well. Autoimmune disorders are more common in women than in men, and they may run in families. Autoimmune disorders can affect various parts of the body such as blood vessels, connective tissue, endocrine glands, joints, muscles, red blood cells, skin, and many others.

It is common to have more than one autoimmune disorder at a time. Most are chronic, or life-long illnesses, although they may come and go in flares. Treatment for autoimmune disorders depends on which part of the body is being attacked. For example:

  • A type 1 diabetic whose pancreas has been damaged will need insulin.
  • A person with Hashimoto's whose thyroid has been damaged will need replacement thyroid hormones.
  • Someone with Sjogren's syndrome will need eye drops and mouth rinses to replace tears and saliva.

Many autoimmune disorders of all kinds are treated with immune-suppressing medications, such as corticosteroids (e.g. prednisone) to reduce the effect of the immune system.

Heart Attack

A heart attack is a serious medical event that happens when the blood supplying oxygen to the heart is cut off, usually because of a blockage in an artery. This results in damage or death of part of the heart muscle. The heart can heal, but like any part of the body that was injured it leaves a scar. This scar tissue does not contract effectively, so the ability of the heart to pump is lowered after a heart attack.

A heart attack, sometimes called a myocardial infarction, can happen without warning. However there are certain factors that raise the risk of heart attack. Some of these are:

  • Age
  • Gender (most heart attack sufferers are male)
  • A family history of heart attacks
  • Smoking
  • Obesity
  • High cholesterol
  • High blood pressure
  • Diabetes
  • Unhealthy lifestyle choices (lack of exercise, poor diet, too much alcohol or stress)

Heart attacks typically feel like pain or pressure in the chest that can radiate out to the back, arm, or jaw. Some other symptoms can include fatigue or weakness, shortness of breath, sweating, and nausea.

Treatment aims to increase blood flow to the heart muscle by opening up blocked blood vessels. Medications are sometimes used to dissolve clots in the arteries. Aspirin and other medications such as Plavix can thin the blood and make clots less likely to form. Dilators can widen blood vessels to make room for more blood to flow. Doctors can also use a procedure called catheterization to look at the blood vessels around the heart directly, place stents, or widen vessels using angioplasty. In extreme cases bypass surgery can be used to help blood flow better. After recovery, it is important to lead a healthy lifestyle and take any prescribed medications in order to protect the heart.

Heart Problems

The heart is one of the most important organs in the body. This smooth muscle expands and contracts rhythmically an entire lifetime, pumping blood to the lungs and then to every other cell in the body. When heart problems occur, it becomes difficult for the body to get the nutrients and oxygen it needs via the blood. So while there are a variety of illnesses and disorders that affect the heart, most of them cause weakness, exhaustion and shortness of breath. The most common heart conditions include heart disease, angina, arrhythmia, and valve disorders.

Heart disease is the leading cause of death in the United States for both men and women. This includes coronary artery disease, heart attacks, congestive heart failure, and congenital heart disease. Some conditions, such as genetics, cannot be controlled. But there are many other things one can do to lower their risk for heart disease. Controlling high blood pressure and cholesterol levels, reducing or stopping smoking, exercising more and losing weight if needed, and eating a diet low in sodium can all protect the heart.

Angina is a squeezing type of chest pain that happens when the muscles around the heart don't get enough oxygen. It can be regular or infrequent. Usually, angina is caused by coronary heart disease. However, not all chest pain is angina. Chest pain can also be caused by a lung infection or panic attack, for example, so it is important to have any sudden pain checked.

Arrhythmia is the word for when the heart beats too fast, too slow, or irregularly with skipped beats. It can feel frightening, and depending on the type it can be dangerous, but in most cases arrhythmia is not serious and can be treated. It is very common, especially in older adults.

Heart valve problems can happen in any one of the the heart's four valves that keep blood flowing where it needs to go. Babies can be born with problems in their heart valves, or valves can be damaged by infections. The valves can stiffen and become less mobile, or they can stop closing properly and 'leak' when the heart beats. Valve problems can usually be repaired surgically.

Dr. Dale D. Hirsch is my cardiologist, and he first cardiologist I've ever been too. He is an exceptional provider. He is approachable, and very kind. He listened to my heart concerns and then asked questions and explained things I didn't understand, in a way that I could understand. He recommended tests and got those approved through insurance in record time. I really felt he was advocating so well on my behalf. I feel like I got extremely fortunate to be in his care. In addition, when a test result showed up in My Chart, on the same day a very kind woman from the office called me after hours (!!) to explain the results in a way I could understand them. I am so very, very grateful that Dr. Dale D. Hirsch is my cardiologist. He is outstanding. I'm grateful to the care team that he has supporting him. I am indeed fortunate.

A routine check up turned into an hour plus response to unexpected happenings occurring at the time of the visit. The entire team, Dr., nurses. And others responded in an efficient, immediate manner to find answers and provide a treatment plan for the symptoms they unexpectedly encountered. The entire staff were professional, caring and exhibited a professional team effort which was appreciated! Quite an unexpected for them to go from a routine medical visit to an expanded visit with med changes. Blood draws. To other medical procedures and tests ordered. Very fine over-all experience. Which was appreciated.

Dr. Hirsch was very friendly during my appointment and put my daughter and I at ease right away. He spoke fairly fast but then said he would have all the information he discussed written down for me to take home with me which he did. He seemed very knowledgeable about my condition and reassured me this can be easily treated. Both my daughter and I liked him and look forward to him continuing to treat me. I would recommend him to anyone I know that needs a cardiologist.

Dr Hirsch was inconsiderate and short. He spoke over my questions and wasn't interested in fully answering what I was able to ask. At one point he drew a diagram to illustrate the underlying physiology and when I pointed at the paper to ask questions he literally pulled the paper away and turned it over so that I couldn't ask questions. I was extremely disappointed and upset about my visit and will be getting a new cardiologist as this was a terrible experience.

I had a previous doctor that I liked a lot. He would listen to my concerns and give me options for treatment. The new doctor that I saw recently was very short with me. Talked the whole time and left me little room to talk or share my concern. I left my appointment feeling more worried and concerned that I was not receiving the care I needed. The rest of the staff, nurses and reception are very sweet kind people I felt completely seen by.

I appreciate being able to talk to Dr Hirsch virtually after experiencing a horrendous storm and continuing to have to deal with it. It is very hard to get an appointment with Dr Hirsch. He is booked solid and being in the waiting list doesn't help. Don't need to see often though. His staff always follow up by the next day and give me good info through Dr Hirsch if I don't tLk directly to him. I get Excellent care and recommend it highly.

I made an appointment with my regular doctor about a problem I had been experiencing in my chest while exercising. He was not able to give me a conclusive diagnosis, but was willing to refer me to a cardiologist. When I called Dr. Hirsch's office I was able to get an appointment in about 2 months. Everyone in that I encountered at the Heart Clinic was helpful and kind, from the receptionist, the nurse, the Doctor and even the scheduler.

Thank you to Dr. Hirsch for taking the time with me. I came in with many questions, it was my 2nd time seeing him after my heart attack. He took the time and explained things to me that was easier for me to understand. Some of the information was technical due to my cardiac situation but he explained it in a way that wasn't over my head, I really appreciated that. He answered the questions my wife had as well. Thank you again.

Dr Hirsch is very knowledgeable and caring... I justcwish at times he would slow down in explaining. I know it routine for him. So I understand that. But I would feel better if he slowed down his explanations a little. But I still appreciate his concern and knowledge of my issues. Thank you Dr Hirsch

Dr. Hirsh was very personable with good humour as well as professional & explaining things well. I felt at ease with him as he appeared very knowledgeable & experienced. I am already going to suggest to a very close friend who had a severe heart attack a few years ago to go to him.
