Joint Injections

A joint is any area of the body where two bones connect. Due to injury or disease, the space between the two bones can sometimes become swollen and inflamed, which leads to pain and a loss of mobility. Injection and aspiration are two tools that physicians use to treat joint pain locally without needing to perform surgery.

Both injection and aspiration are techniques that involve inserting a needle connected to a syringe directly into the joint. Aspiration involves the removal of excess fluid, and injection is the placement of medication directly into the joint space via the needle. Both procedures may be performed at the same time. In both cases a local anesthetic may be used, the skin will be cleaned and disinfected, and then the needle will be inserted. In some cases, especially if the injection or aspiration is in a large and deep joint such as the hip or spine, ultrasound may be used to guide the needle to the exact location desired.

In some cases, irritation to the joint can cause fluid to build up so significantly that the joint hurts and can no longer move well. Aspiration removes some of the excess fluid and relieves the pressure. In addition, aspiration can be used to provide a sample of joint fluid if it needs to be examined microscopically for the presence of white blood cells, bacteria, or crystal formations.

Certain injuries and diseases that affect joints are inflammatory in nature, such as rheumatoid arthritis, tendonitis or gout. These diseases may be helped by the local injection of anti-inflammatory medications directly into the affected joint. In this case, corticosteroids such as methylprednisone are usually used. Relief from the pain may be felt right away and may last for weeks or even months.

In some cases, such as with osteoarthritis, the cartilage buffer between the two bones wears down and pain comes from bones rubbing against each other. In this case, injection of a lubricating agent such as hyaluronic acid may be beneficial. It provides a slippery cushion between the bones to relieve pain that can last for months.

Musculoskeletal Problems

The musculoskeletal system refers collectively to the bones, joints, ligaments, muscles, and nerves. It is what gives the body structure and movement. Because this system encompasses so much of the body, musculoskeletal problems are extremely varied and can happen almost anywhere. There are musculoskeletal problems that affect only the joints, those that affect the bones, those that affect tendons and ligaments, and those that can happen in any area of the body but cause pain and numbness.

Joint problems include bursitis and arthritis. Bursitis is the inflammation of a fluid-filled sac cushion on the outside of a joint, causing symptoms including pain and swelling. Arthritis is the inflammation and damage of a joint due to wear and tear or disease. There are several treatments for arthritis, but if the damage progresses far enough, joint replacement may be necessary. In joint replacement, a metal or plastic implant is surgically placed within the joint to make movement easier.

Bone problems include scoliosis, fractures, and osteoporosis. Scoliosis is a curvature of the spine, making the normally straight spine look like a 'C' or 'S.' It is treated with braces or surgery. Osteoporosis is a disease that causes bones to weaken and break easily. It's most common in women, especially older women. Fully half of all women over 65 have osteoporosis. Fractures are any break in the bone. They can be a simple crack or a severe and complicated shatter. Osteoporosis causes fractures in people who have it, but other causes are trauma (such as a fall) or overuse.

Tendon and ligament problems most often result from injury or overuse. Two good examples are ligament tears and tendonitis. The most common ligament tear is the ACL tear in the knee. This ligament supports and stabilizes the knee and is most often torn during sports activities. Tendonitis happens when a tendon, which connects muscles and bones together, becomes irritated and inflamed. This happens most often in older patients who push their bodies too far, leading to pain and swelling. Treatment for both tendon and ligament problems usually includes rest, ice, and supporting the area to let it heal.

Pain and numbness problems can be caused by overuse, disease, an injury, or a compressed nerve. Lower back pain and carpal tunnel syndrome are two of the most common conditions patients encounter. Lower back pain may be caused by sore muscles that have been overworked, or an injury to the disks separating the vertebrae in the spine. Lower back pain usually goes away on its own within a few days, but it may require medical treatment depending on the cause. Carpal tunnel syndrome occurs when the nerve at the base of the palm becomes trapped or pinched. Symptoms include tingling, pain, numbness, or weakness in the hand. Treatment involves rest and sometimes steroid injections.

Because the musculoskeletal system involves so much of the body, problems here can be incredibly varied in their type and severity. Some musculoskeletal problems will go away on their own or only require rest, while others may require medication, physical therapy, or even surgery.

Replacement Arthroplasty (Joint Replacement)

Joint replacement, sometimes also called arthroplasty, is an option when a joint becomes severely damaged by disease or injury. The damaged cartilage of the joint is surgically removed, the ends of the bones in the joint are resurfaced, and a prosthetic is installed. Most prosthetic joints are made of a metal piece that fits into a plastic sleeve so that they glide smoothly. A joint replacement increases stability in the damaged area and decreases pain. The hip and knee are the two joints most commonly replaced, but joint replacement can also be performed on the ankle, shoulder, elbow and even fingers.

Although joint replacement is one of the safest and most reliable medical procedures available, it is still a major surgical procedure. All surgical procedures carry risks, such as infection or blood clots. For this reason, doctors try to help their patients avoid surgery for as long as they can. Taking arthritis medications, losing weight, avoiding high impact sports such as running, taking supplements such as glucosamine or chondroitin, or having joint injections may allow a patient to enjoy life without the need for surgery.

After having joint replacement surgery, patients can expect some pain the first few days as they recover. Physical therapy is an important part of recovery, and it can decrease complications and increase future mobility. As soon as possible, returning to low impact sports such as swimming, walking or biking can stretch and heal the new joint. A prosthetic joint can last 15 - 20 years and cannot be further damaged by degenerative diseases such as arthritis, so once it is done patients should be pain-free for many years.

Dr. Chad Marion was very knowledgeable about ortho-surgery, he is on top of my existing injury condition, he was extremely logical, rational, as well as sequential, without forgetting methodological.He explained my MRI results to me in a manner that, The easy ones gets explained first, Then move on to the more difficult ones, Then the final subject on what he saw that requires a surgery.And then, he gave a lot of room for doctor/patient Q&A discussions, And provided his expertise experiences as references.It was a great surgical consult session, Dr. Chad Marion has so much confidence, and all the reviews from patients who considered their surgery results a success, I have a lot of confidence in him.Simply put, he's a great doctor.

I was referred to this doctor for a consultation on my right knee. There was a bit of a digital glitch initially when I arrived at the exam room. The medical assistants were very helpful and worked with what was available to them. The entire experience was an example of the quality of employees that Pacific Medicat Centers have. I was very impressed. My interaction with the doctor was another example of the quality of doctors that work with Pacific Medical Centers. All of my questions and concerns were answered.

I am very happy with this practice. One frustration was that I had some difficulty getting an appt same day following a fall with a resultant fracture. It eventually did work out that I got seen, but getting same day urgent care was not easy, and call backs were kind of slow. It did work out so I did not have to go to ER or an urgent care facility. Much better to be seen at pac med.Really appreciate having doctor, xray, radiologist, etc in 1 place.

Dr. Marion has performed a miniscus surgery on my left knee & is most likely my shoulder replacement surgeon when the time comes (we have already consulted). This visit last week was for my right knee, pain possibly from my hip, which is being replaced in December. As it wasn't an emergency, I was satisfied with the very short wait time to get in to see him. He had x-rays taken & answered all my questions. I enjoy his professionalism & knowledge.

Just a good all around visit regarding a referral to orthopedics, referred by my primary card doc after a shoulder dislocation. While it would have taken a while to get in to the closest PacMed facility, the scheduling person was very helpful making sure I DID get in to see Dr Marion. And was again helpful and accommodating when we needed to change the day due to weather and transportation issues. And Dr Marion was kind, concerned and helpful.

I was late, so my appointment was a little later. I felt it was a bit short, but all my questions were answered. Dr. Marion has done all he can do for me at this point. He has referred me to a Dr who can do total knee replacement for me. That Dr will explain more about the procedure to me. I am completely Satisfied with all of my medical team at PacMed.

I've always found it helpful to be polite and persistent If necessary to get the desired appointment. In my case, the appointment desk wanted to send me to my PCP for an xray on my foot whereas I wanted to see an orthopedic provider asap. Thankfully everything worked out perfectly as I was able to get the appointment along with the providers I needed.

Dr. Marion is an outstanding physician whom I would unequivocally recommend to others. He rates in my mind as a "best doctor" in Seattle. Took care of my problem and made Important recommendations about future care needed. Staff (front office and back office) was also courteous, understanding and very helpful.

I really have not had any bad experiences. I really like my primary care doctor and Doctor Marion was seen for Knee problems I have. It has been about two years since my last visit with him. He has ALWAYS been great with my care. I have nothing but good things to say about him.

Dr. Marion was most helpful in providing a second opinion regarding shoulder replacement. He listened to my somewhat unlikely theory and offered a way to test in to determine if they're was any credibility to it. I am very grateful for his patience and thoughtful suggestions.
